For your convenience, you may print our new patient form and complete it prior to your appointment. The form is in Adobe Acrobat (.pdf) format and is viewable and printable with Acrobat Reader, which is a free program.

If you do not have Acrobat Reader, you may download it from Adobe's website.

Your First Visit
Your first visit will include a comprehensive oral examination with the appropriate radiographs. We are equipped with both an intra-oral camera and a digital camera to image your new smile. We will discuss your needs and wants with possible treatment options to personalize your treatment plan. You will take home your personal treatment plan on a computerized printout, including fees and insurance breakdowns. If your treatment plan is extensive, this may be done in two visits.

After you have been properly diagnosed and your treatment planned, we use our CAESY DVD for educational purposes. We feel that one cannot fully appreciate the value of our services unless they are understood. It is through education that we may prevent oral disease and restore you to optimal oral health.

Patient Safety
To protect our patients and ourselves, our staff wears masks, gloves, glasses and disposable uniform jackets. We strictly observe all OSHA guidelines. All instruments are cleaned, disinfected and routinely sterilized as recommended by the American Dental Association and Centers for Disease Control. Whenever possible, disposable single-use items are used to ensure your safety. Hand pieces (drills) are sterilized with state-of-the-art sterilization units. All surfaces in the treatment rooms are cleaned and disinfected after each patient.

Hygiene
An integral part of maintaining your teeth for life is the removal of plaque and tartar on a regular basis. The frequency of your visits is based upon your oral health and individual needs. During your visit, we will update your medical history, perform an oral cancer screening exam, check your teeth for cavities, review oral hygiene instructions, and monitor your periodontal (gum) condition. Fluoride treatments are provided for children under the age of thirteen and for adults when appropriate.

Patient Comfort
You are not alone if you feel anxious about a visit to the dentist. We understand and share your concerns and strive to make every dental visit comfortable for our patients. In addition to topical and local anesthetics, nitrous oxide is available for those patients who may need it. If there is anything else we can do, please let us know.  We are on call 24 hours a day for emergencies.

Fees and Insurance
Our fees are based on the care, skill and judgment necessary to provide optimal dental care. You will be given an explanation of costs and time involved for your treatment. We will assist you with financial arrangements if they are needed. We will also gladly file your insurance claims, if applicable. We accept all major credit cards. Care Credit and Dental Fee Plan financing options are also available. Please do not hesitate to ask any questions about your benefit plans, our services or our fees.
